This form is used by a judge to approve a request from someone who cannot afford to pay court fees, allowing them to proceed with their case without paying filing fees or other court costs. A person would need this form if they have low income or limited money and want the court to waive (skip) the fees that are normally required to file a lawsuit or other court case.
This form must be printed and filled by hand, or opened in Adobe Acrobat.
