# ADR-402 (INFO) Form The ADR-402 form is used to provide basic information about a dispute and the people involved when someone wants to try mediation or another alternative dispute resolution method instead of going to trial. Anyone filing a case in Contra Costa Superior Court who wants to explore settling their disagreement through mediation would need to complete this form.
