Pleading on Joinder—Employees Benefit Plan

This form allows someone to join an employee benefit plan as a party in a family law case so that the plan can be directly involved in decisions about dividing retirement benefits or other plan assets. Family law attorneys and litigants use this form to make sure an employee benefit plan receives notice and has the opportunity to respond to claims about benefits that belong to the plan or one of its members.
