The SV-800 form is used to document that someone has safely disposed of their firearms by turning them in to law enforcement, selling them to a licensed dealer, or storing them with a licensed facility. A person would need this form if they are subject to a court order (such as a domestic violence restraining order or gun violence restraining order) that requires them to give up their guns, and they need to prove to the court that they have complied with that requirement.
