This form is used in juvenile dependency cases to declare that you have notified the other party (or explain why you couldn't) before asking the judge to make a temporary decision without them being present in court. A parent, social worker, or attorney involved in a child welfare case would use this form when they need emergency court action and must document their efforts to give the other side notice beforehand.
