This form is used when two parties in a court case agree to work with a private mediator to try to settle their dispute outside of court, rather than having a judge make the decision. Either party in a civil lawsuit (such as a divorce, contract dispute, or personal injury case) who wants to attempt resolving their case with professional help would use this form.
