This form is used when someone asks a judge for an order without the other person being present in court, and they need to explain in writing why the other person shouldn't be notified ahead of time. Someone would need this form if they're requesting an emergency court order (like a restraining order or temporary custody arrangement) and believe that giving the other person advance notice would cause harm or defeat the purpose of the order.
