This form is used to officially end a conservatorship, which is a legal arrangement where one person (the conservator) has been managing the personal care and/or finances of another adult who cannot do so themselves. A conservator or family member of someone under conservatorship would use this form when the person is no longer in need of this protection—for example, if they've recovered their ability to manage their own affairs or have passed away.
