This form allows a person who won a small claims court case to create a written payment plan with the person who owes them money, where the debt gets forgiven if the debtor makes all the agreed-upon payments on time. A judgment creditor (the winner of the case) would use this form if they want to collect their money gradually through installments instead of demanding full payment immediately or pursuing other collection methods.
