This form is used by parents in a custody or family law case to agree together that a neutral third party (called a parenting coordinator) will help them resolve disagreements about parenting issues, schedules, and their children's care. Parents who are having trouble communicating or making decisions together about their kids would use this form to ask the court to approve and hire this coordinator to work with them.
