This form is used to officially document that a court has granted someone temporary authority to make decisions for and care for another person who cannot care for themselves. A person would need this form if they've been appointed by the court to temporarily look after a child, elderly adult, or someone with a disability while a permanent guardianship or conservatorship arrangement is being decided.
