The Mental Health Packet is used to file a court case when someone needs to be involuntarily held or treated for a mental health crisis, or when a person wants to challenge such a hold. A family member, healthcare provider, or concerned person would use this form to petition the court for a mental health evaluation or commitment order for someone they believe is a danger to themselves or others.
