Form FL-120 is used to tell the court that you've notified the other person about your request for emergency court orders, or to explain why you couldn't notify them before asking for the orders. Someone would need this form if they're asking a judge for immediate protection or custody decisions in a family law case and need to prove they tried to give the other person notice, or document why advance notice wasn't possible.

This form must be printed and filled by hand, or opened in Adobe Acrobat.
