This form is used when someone who owes a court judgment stops making their required installment payments, and the creditor wants the judge to cancel the original payment plan agreement. A creditor (the person or business owed money) would use this form to ask the court to declare that the debtor has violated their payment agreement and to get a new court order requiring full payment instead of installments.
