The Pre-Trial Order Form is used to organize and document important details about a case before it goes to trial, including what facts both sides agree on, what they disagree about, what evidence will be presented, and what legal issues need to be decided. Attorneys, judges, or people representing themselves in civil lawsuits would use this form to prepare their case and make sure everyone knows what to expect at trial.
This form must be printed and filled by hand, or opened in Adobe Acrobat.
