This form is used to request that the court assign a special advocate to represent a child's best interests in court cases, or to appoint someone to make educational decisions for a child who needs support. A parent, guardian, social worker, or other person involved in a child's case would use this form when they believe the child needs an independent adult to help protect their rights and welfare.
