The Packet Cover Sheet (MMC-100) is used to organize and label the documents you're submitting to Mendocino Superior Court so the court can properly process and file your case paperwork. Anyone filing documents with the court—whether you're starting a lawsuit, responding to one, or submitting other official papers—would need to fill out this cover sheet to ensure your materials are clearly identified and routed to the correct department.
