Notice of Conservatee's Rights

Nevada Superior Court · Probate

This form tells someone under conservatorship what legal rights they still have, such as the right to make decisions about their own medical care, money, or personal life. A conservator (usually a family member or court-appointed person) must give this notice to the conservatee (the person being cared for) so they understand they're not losing all their rights just because someone else is helping manage their affairs.
