This form is a cover sheet that goes on top of a packet of court documents to organize and identify what papers are being filed with the court. Someone filing their own case without a lawyer would use this form to list all the documents they're submitting so the court clerk knows exactly what's included in their packet.
This form must be printed and filled by hand, or opened in Adobe Acrobat.
