Child Habitually Disobedient § 601(a)

This form is used to document that a child is habitually disobedient, ungovernable, or beyond parental control in order to establish grounds for juvenile court intervention. Probation officers, social workers, and parents use this form to petition the court for supervision or custody modification of a minor who repeatedly violates family rules or refuses parental discipline.
