This form is used to request that a child be held in custody by the court while a dependency case (involving child welfare concerns) is being decided, and to notify the parents or guardians about this detention hearing. A social worker, child protective services worker, or law enforcement officer would use this form when they believe a child needs immediate protection and court approval to be removed from their home.
This form must be printed and filled by hand, or opened in Adobe Acrobat.
