This form is used to officially reduce or dismiss certain low-level criminal charges (like shoplifting, drug possession, or theft under $950) that were previously convicted as felonies, converting them to misdemeanors instead. A person who was convicted of one of these crimes in the past and wants to have their conviction reduced to a lower level would need to file this form with the court.
