This form allows both parties in a civil lawsuit to agree that a neutral mediator will help them work out their dispute instead of having a judge make a binding decision through arbitration. Someone involved in a lawsuit who wants to try settling their case through a mediator's help rather than going through the court's arbitration process would need this form.
