# SIE-0042: Appointment, Termination, or Resignation of Supporter This form is used to officially add, remove, or step down from a court-appointed support role in a case, such as a guardian, conservator, or similar position that helps manage affairs for someone who needs assistance. The person using this form would be someone who is either being appointed to help manage someone else's needs, wants to quit that job, or needs to remove someone who is currently serving in that role.
This form must be printed and filled by hand, or opened in Adobe Acrobat.
