The FL-157 form is used to provide detailed financial information and explain the circumstances when someone is asking a court to decide on spousal or partner support payments in a family law case. Either spouse or partner in a divorce, separation, or domestic partnership dissolution case would need to fill this out to help the judge understand their income, expenses, and reasons for requesting support.
