# Infraction Request Form (SB-16350) This form is used to request a court hearing for a minor traffic or non-traffic violation (called an infraction) when you disagree with a ticket you received or want to contest it in court. A person would need this form if they received a citation for something like a traffic violation or other minor offense and want to formally appear before a judge rather than just paying the fine.
This form must be printed and filled by hand, or opened in Adobe Acrobat.
