The MFL-103 is a cover sheet that organizes and identifies all the documents you're submitting together as one complete package to the court. You would need this form if you're filing multiple related court papers at the same time, such as when starting a case or submitting several documents as part of a single request.
This form must be printed and filled by hand, or opened in Adobe Acrobat.
