The MUD-101 is a cover sheet that organizes and identifies all the documents you're submitting together as one complete filing package to the court. Someone filing a case or submitting court documents would use this form to list what papers are included so the court clerk can make sure nothing is missing.
This form must be printed and filled by hand, or opened in Adobe Acrobat.
