The DV-101 form is used to ask a court for a restraining order (a legal order to stop someone from contacting, threatening, or coming near you) when you are being abused, threatened, or harassed by someone you have a close relationship with, such as a spouse, partner, family member, or roommate. You would need this form if you are experiencing domestic violence and want the court to protect you by ordering the other person to stay away from you.
