This form is used to officially notify the court and other interested parties that someone is applying for a certificate that would reduce the negative effects of a criminal conviction on their record. A person who has completed their sentence and probation for a felony conviction would use this form to inform the court of their request to have their conviction treated more favorably in employment and licensing decisions.
This form must be printed and filled by hand, or opened in Adobe Acrobat.
