Request for Removal of Co-Guardian

This form is used when someone wants to ask the court to remove a co-guardian (a person who shares guardianship responsibility) from their position because they believe that person is not properly caring for the child or handling their finances. A parent, relative, another co-guardian, or the child themselves (if old enough) would use this form to request the court's help in removing the unsuitable co-guardian.
