This form allows a teenager in foster care or under court supervision to agree that a Court Appointed Special Advocate (CASA) volunteer can be assigned to represent their interests and help protect their rights in court proceedings. A non-minor dependent—typically someone aged 18-21 who is still in the child welfare system—would need to sign this form to give permission for a CASA to work on their behalf and to allow information about their case to be shared with the advocate.
