The CR-100A form is used to evaluate a person arrested for drunk or drugged driving to determine what treatment or counseling programs they may need. Someone convicted of a DUI charge in Contra Costa County would need to complete this form so the court can decide on appropriate penalties and rehabilitation requirements.
