This form is used by two people involved in a civil lawsuit who agree to try alternative dispute resolution (like mediation or arbitration) instead of going to trial to settle their disagreement. Either party to a civil case would need this form when they both want to explore a faster, less costly way to resolve their dispute outside of court.
