This form is used when someone accused of a crime agrees to participate in a mental health treatment program instead of going through the regular criminal court process, with the goal of having their charges dismissed if they complete the program successfully. A person would need this form if they have a mental health condition that contributed to their criminal charges and they want the opportunity to get treatment and avoid a criminal conviction.
