The UD-100 form is used by a landlord to start a court case to evict a tenant from a rental property. A landlord would need this form when a tenant stops paying rent, violates the lease agreement, or refuses to leave after their tenancy ends.
This form must be printed and filled by hand, or opened in Adobe Acrobat.
