This form is used when someone wants to move a conservatorship case (where a court appoints a person to manage affairs for someone who can't do it themselves) from Mendocino County to a different county. A conservator, conservatee, or interested family member would use this form if there's a good reason to have the case handled in another county, such as if the conservatee has moved or most witnesses live elsewhere.
