The EA-800 is used to request a judge's decision on whether someone should be declared mentally incompetent to manage their own finances and personal care. A family member, caregiver, or concerned person would need this form when they believe an adult (usually elderly or seriously ill) can no longer make safe decisions about money or medical treatment and needs a court-appointed guardian or conservator.
