The FL-105 form is used in child custody cases to tell the court where a child has lived for the past five years and whether there are any other court cases involving that same child. A parent or guardian involved in a custody dispute in Shasta Superior Court would need to fill this out to help the court understand the child's history and avoid conflicting rulings from different courts.
