The APP-103 form is used by someone who lost a case in Imperial Superior Court and wants to appeal the decision to a higher court; they use it to tell the court which documents and records from their original case should be included in the appeal. A person who received an unfavorable judgment and believes the lower court made a mistake would need to file this form as the first step in appealing their case.
