The PSC-21 form is used to request an official document from the court clerk proving that a judgment (a court's monetary decision) has been paid off or otherwise satisfied. Someone who borrowed money, lost a lawsuit, or had a judgment against them would use this form to get proof from the court that they have fulfilled their obligation.
