# SIE-0045 Purpose and Use This form is used when someone pleads guilty to a drug-related offense but agrees to complete a drug treatment program instead of going to trial, with the possibility of having the charges dismissed if they successfully finish the program. Someone struggling with drug addiction who is charged with a drug crime would need this form to enter into that treatment agreement with the court.
This form must be printed and filled by hand, or opened in Adobe Acrobat.
