This form is used to quickly appoint someone to make decisions for a person who cannot care for themselves (like a child without parents or an elderly adult who is incapacitated) while waiting for a permanent arrangement to be finalized. A family member, friend, or social worker would use this form when they need immediate legal authority to handle a vulnerable person's care or finances before a full guardianship or conservatorship hearing can take place.
