The Mental Health Diversion Screening Form is used to determine whether someone accused of a crime is eligible for a mental health treatment program instead of going through the regular criminal court process. This form would be used for people who have committed crimes but have mental health conditions that contributed to their criminal behavior, and who might benefit from treatment rather than jail or prison time.
This form must be printed and filled by hand, or opened in Adobe Acrobat.
