This form is used to document that someone is actively looking for a job by listing the employers they've contacted, job applications they've submitted, or other job-seeking activities they've done. A person receiving unemployment benefits, welfare assistance, or involved in a court case (such as child support) would need to complete this form to prove they're making genuine efforts to find employment.
