This form allows people who cannot afford the court fees to have those fees removed or reduced when they are asking the court to appoint them as a guardian or conservator for someone else. Someone would need this form if they want to become a legal caretaker for a child or an adult who cannot care for themselves, but they do not have enough money to pay the court costs.
