The PADR-01 form is used by people involved in a civil lawsuit who agree to try alternative dispute resolution (like mediation or arbitration) instead of going to trial to resolve their case. Either party in a civil dispute—such as someone suing over a contract disagreement, property damage, or money owed—would use this form to formally tell the court that both sides want to attempt settlement through a neutral third party.
