This form is used when an adult wants to be legally adopted by another adult, which transfers parental rights and responsibilities and creates a legal parent-child relationship. Someone would use this form if they have an adult child (typically someone 18 or older) who wants to formalize a family relationship through adoption, such as when a stepparent adopts a grown stepchild or when an adult wants to adopt someone they've raised.
