The JV-667 form is used in child welfare cases to document the court's decision about where a child will live when they are removed from their parents' care, such as with a relative, in foster care, or in a group home. A social worker, child protective services agency, or attorney involved in a dependency case would need to complete this form to inform the court and all parties about the child's placement arrangement.
