This form is used to prevent the court from releasing private records from a juvenile's case—either a delinquency case (involving a minor accused of a crime) or a dependency case (involving a minor in foster care or child protective services)—to someone who has requested access to those records. A parent, guardian, the juvenile themselves, or their attorney would use this form if they want to keep the case file confidential and prevent it from being given to the person who asked for it.
